When is a lifting lever required per ASME code?

ASME Code requires a lifting lever for Air, Steam, and Hot water (140F+) specifically. Any other fluid does not require a lifting lever. However, there is code case 2203 which the end user can invoke which waives the requirement for a lifting lever.
